Does anyone know where to locate this form required by SEF for the main GV applicant?
"Filling in the Application Form (standard form) stating the authorisation for consultation of
the Portuguese Criminal Record"

I didn’t see any such standard form. There was only one standard form in my doc stack. There was something that looked like it but it was free-form.

Thanks Jeff, I have actually found the standard form on the SEF website here:

It includes the required wording about Portuguese Criminal Records.
